By providing my credit card information above, I am agreeing to the following terms and conditions. I hereby authorize NAMI Solutions, Inc., and its successors and assigns to initiate charges to my credit card indicated above. This authority shall remain in effect until NAMI Solutions, Inc. has received a thirty (30) day written notification from the cardholder of its termination in such time and manner as to afford NAMI Solutions, Inc. a reasonable opportunity to act on it, or until NAMI Solutions, Inc. has sent the cardholder ten (10) days written notice of NAMI Solutions, Inc. termination. If I choose to terminate this authorization to my credit card account, I will notify NAMI Solutions, Inc. in accordance with the above. I understand NAMI Solutions, Inc. will notify me in writing of any changes in the dollar amount being charged to my account.
